DATA PRIVACY POLICY AND TERMS
OF AGREEMENT for Students
I.
Introduction
St. Theresa's College of Cebu is committed to
protecting the sanctity of personal information and the rights of individual
students to Data Privacy as stated in the Republic Act No. 10173, also known as
the Data Privacy Act of 2012 (DPA), which aims to protect personal data in
information and communications systems both in the government and the private
sector.
STC of Cebu intends to act in accordance with
Data Privacy Act of 2012 (DPA) and collaborate fully with the National Privacy
Commission (NPC). STC of Cebu ensures that all entities processing personal
data establish policies and implement measures and procedures that guarantee
the safety and security of personal data under its control or custody, thereby
upholding the student's data privacy rights. This Privacy Policy informs the
student about the school's policy concerning the data that the school collects,
utilizes, discloses and transfers including his/her personal data. In this
policy, the terms data and information
are used interchangeably. The term, personal data, includes the personal
information, sensitive personal information, and privileged information.
This Data Privacy Policy and Agreement Form
may be revised from time to time without prior notice, and such revisions will
be known to you through our STC of Cebu website or by email.
II.
What kind of information does STC of Cebu
collect, acquire and generate?
STC of Cebu collects, acquires and generates personal data from its
present, previous and future students. The student's data are collected upon
enrolment and during their entire stay at the school. This data may consist of
written records, photographic or video images, and digital file. The data
collected may entail the following:
✓ full name, address, contact numbers and other contact details;
✓ birth date and place of birth, nationality, immigration status,
religion,
✓ email address;
✓ family history including the data on parents, guardians and siblings;
✓ academic performance and disciplinary record of student from the
previous school;
✓ health or medical record, psychological assessment records, disciplinary
records, and physical fitness information;
✓ data collected on student's academic and co-curricular performances and
activities;
✓ Guidance Cumulative Records which consist of interviews or conferences,
exam results, behavioral information, and other guidance assessments;
✓ Student's Permanent Records including transcripts and the academic
record of the student in STC of Cebu
✓ Socio-economic profile of student's family;
✓ ITR of parents for ESC (Education Service Contracting) requirements; and
✓ Billing and other financial information.
III.
How will STC of Cebu use the student's
information?
The collected, acquired and generated information is used exclusively
for the following purposes:
✓ evaluating
applications for admission to the school;
✓ processing
confirmation of incoming or transfer students in preparation for enrollment;
✓ recording,
generating, and maintaining student records of academic and co-curricular
performances;
✓ recording,
storing, and evaluating student output such as assignments, seatwork, quizzes,
long tests, summative exams, term papers, theses, research papers, reflection
journals, essays, presentations and other performance tasks;
✓ recording,
generating, and maintaining records, whether manually, electronically, or other
means, of class attendance and participation in curricular and co-curricular
activities;
✓ establishing
and maintaining student information systems;
✓ sharing of
grades between and among faculty members, and others with legitimate official
need, for academic deliberations and evaluation of student performance;
✓ processing
scholarship applications, grants and other forms of financial assistance;
✓ investigating
incidents that relate to student behavior and implementing disciplinary
measures;
✓ maintaining
directories and alumni records;
✓ compiling
and generating reports for statistical and research purposes;
✓ providing
services such as health, insurance, counseling, information technology,
library, sports/recreation, transportation, parking, campus mobility, safety
and security;
✓ managing and
controlling access to campus facilities and equipment;
✓ communicating
official school announcements;
✓ sharing
marketing and promotional materials regarding school-related functions, events,
projects, and activities;
✓ soliciting participation in research and non-commercial
surveys;
✓ posting of
academic and co-curricular achievements within the school premises and/or
website; and
✓ for
accreditation, professional development of faculty and staff, and research like the
action research done by the teachers for the improvement of instruction.
IV.
How will STC of Cebu share, disclose or
transfer student's information?
STC-Cebu
may share, disclose or transfer student's information for purposes such as:
✓ posting
of official class lists and class schedules in
the class room and school bulletin boards or other places within the
campus;
✓ providing
of information to persons, including parents, guardians or next of kin, as
required by law or on a need-to-know basis as determined by the school to
promote the student's best interests, or protect his/her health, safety and
security, or that of others;
✓ providing
academic institutions, companies, government agencies, private or public
corporations, or the like, upon their request, with scholastic ranking
information or certification of good moral character for purposes of admission;
✓ giving information to potential donors, funders or
benefactors for purposes of scholarship, grants and other forms of assistance;
✓ distributing
the list of graduates and awardees during
completion ceremony and commencement exercises;
✓ reporting
and/or disclosing information to the NPC and other government bodies or
agencies like Department of Education;
✓ sharing
information for accreditation and school ranking purposes like Philippine
Accrediting Association of Schools, Colleges and Universities;
✓ answering
or responding to inquiries verifying that he/she is a bona fide student or
graduate of the school;
✓ conducting
research for purposes of institutional school development;
✓ sharing
directory information to the STAA(St. Theresa's Alumni Association);
✓ complying
with court orders and /or other legitimate interest;
✓ publishing
academic and co-curricular achievements and success, including honors lists and
names of awardees in school bulletin boards, website, social media sites and
publications;
✓ sharing academic accomplishments or honors and
co-curricular achievements with schools
the student graduated from or was previously enrolled in, upon their request;
✓ marketing
or advertising to promote the school, including its activities and events,
through photos, videos, brochures, website posting, newspaper advertisements,
physical and electronic bulletin boards, and other media;
✓ publishing
communications with journalistic content, such as news information in STC of
Cebu's publications like The Catalyst,
Star, and Yearbook , social media sites and news and media
organizations; and
✓ live-streaming
of school activities and events.
V.
How will STC of Cebu store and retain your
information?
STC-Cebu
will retain student's relevant personal data for an indefinite period for
historical and statistical purposes. The retention period is provided by law
and/or a school policy, all affected records will be securely disposed of after
such period.
Students' personal data are stored and
transmitted securely in paper and electronic formats, including databases that
are shared between the school's different units or offices. Access to student's
personal data is limited to a school personnel who has a legitimate concern in
the student for
the purpose of carrying out his/her duties. The school will not allow excessive
use of the student's personal data.
VI.
How will STC of Cebu handle data breach?
STC of Cebu
recognizes the student's rights with respect to his/her personal data, as
provided by the DPA. If he/she chooses to exercise any of his/her rights, or if
he/she has questions about how the school processes personal data, the school
will consider his/her request and reserves the right to deal with the matter in
accordance with the law.

student have concerns regarding his/her privacy rights, this Agreement Form or

Agreement
Form
I have read St. Theresa's College of Cebu
Data Privacy Policy and express my consent for the school to collect, utilize,
disclose and transfer my personal data articulated in the policy for the
purpose of school administration, and for STC of Cebu to meet its statutory
obligations to its students and graduates.
